This project evaluated the effectiveness of knowledge translation activities in the dissemination and uptake of two deliverables from a project with nine

SPHERUFirst Nation and provincial communities in Northern  the community-based health and socialprograms in place at the time. The combined impact of locally delivered health programs on community health. The KT study identified how the Community Health Indicators Toolkit and logic models were being used by  community partners and outside organizations, how widely they had been distributed, and what recipients wanted to know about their work.
